When we receive this form we will send you a confirmation letter indicating the details of your direct debit donation with National Maternity Hospital NMH Foundation Ltd. You will also receive an email immediately acknowledging receipt of this form.

Note: your direct debit will be taken on the 2nd day of each month or next working day.

By signing this mandate form, you authorise (A) National Maternity Hospital NMH Foundation Ltd, to send instructions to your bank to debit your account and (B) your bank to debit your account in accordance with instruction from National Maternity Hospital NMH Foundation Ltd.

As part of your rights, you are entitled to a refund from your bank under the terms and conditions of your agreement with your bank. A refund must be claimed within 8 weeks starting from the date on which you account was debited. Your rights are explained in a statement that you can obtain from your bank.

By signing this mandate form, you authorise National Maternity Hospital NMH Foundation Ltd to provide at least 4 days advance notice before the first direct debit is collected from your account.
